l work at a surfing company & over hear conversations.
Ussually the athlete & the sponsor agree on the following:
The term, how long the contract will last.
The payment for the athlete, some get product only, some get product & cash. Rumors r that the top WCT guys get $200K+ per year.
Whether or not the sponsorship is exclusive or if the rider can have other sponsors as well. Bonuses for magazine ads, TV / video exposure & contest results.
Also sometimes the riders have a pro model & they get a cut for each unit sold with their name on it.

The endorsement packages from smaller companies r usually not formal. Most companies simply sponsor by providing product, sometimes at cost, & only pay when they use a picture or have u in an ad. For the shortboard pros who make the ASP tour, they r usually provided entry fees, travel money, & for the higher ranked pros, fees based on visibility. Longboard pros have less active financial arrangements. Even the top pros often only have their entry fee & travel paid. l am not sure how much the other know as some of their facts seem way off. My brother has been paid by sponsers since he was 17. The top pros recieve way more than $200,000 a year. Kelly probably gets a 7 figure sposonship deal. And Taj, Andy, Park, & Fanning will be easily in the hundreds of thousands. l guarantee there r no WCT surfers that get only their sponsorship fees paid for. This would only be applicable to average sponsored grommets. The lower pros on the WQS get tens of thousands a year. My brother uses the majority of his sponsorship for travelling, (he is almost never at home). Most of this is travelling around the globe for competition, which u discuss with ur sponsor what u should be doing, but there r rarely any strict rules. There r also long training trips out of season. Then some extra bonuses r given for exposure. He has a certain amount of money his sponsores can give him each year depending of the sq cm he clocks up in magazine pics. The sponsors also provide a few photo shoot trips abroad, & modeling shoots. Hope this helps.
